LTC Unit Coordinator LPN - LTC Administration
The LTC Unit Coordinator assigned to Seward Mountain Haven is a Licensed Practical Nurse. The Unit Coordinator provides administrative support to the LTC unit as request by the Nursing Director by performing various clerical tasks to include appropriately and accurately maintain resident medical records and unit records (both electronically and on paper). Collaborates with team members to coordinate unit activities and flow of information between teams and other entities. This work involves assisting Nurse Managers and Clinical Staff by answering inquiries, referrals and physician's orders over the telephone. Assesses incoming information and coordinates through proper routes. The Unit Coordinator provides assistance to the Medical Providers for coordination of care for all LTC Residents. Maintaining the physician rounding schedule, scheduling appts, & pharmacy correspondence. Coordinates transports for off-campus medical appointments.
Graduate of a School of Nursing
Licensed Practical Nurse (LPN) with Current Alaska licensure
Current American Heart Association (AHA) - Basic Life Support (BLS)
- Two (2) years experience in Long Term Care setting

Providence Mountain Haven is the first Green House® home in Alaska, which is includes 4-lodges, each with 10-Elders. Teams are self-managed, have consistent staffing, and have strong patient to caregiver ratio. Caregivers typically have 5:1 patient ratio and work 12-hour shifts. Nestled against Iron Mountain, in Seward, AK, it is a great home for Elders to live out their later years!
