In this position you will:

The Medical Social Worker provides a wide verity of administrative and clerical support to the Social Service department and other departments in accordance with Providence philosophy, objectives, polices procedures and standards. The Medical Social Worker is responsible for identify and resolve the medically related social and emotional needs of the resident, patient, and client. Fills in as needed for the Medical Social Work Manager to include attending acute discharge planning meetings and swing bed resident care conferences and provides input as to patients financial concerns, discharge needs and plans. Fosters change and teamwork to ensure constructive relationships and growth, while maintaining a positive and professional image when interacting with patients, their families, physical needs.

In this position you will:

  • ENSURING QUALITY OF CARE: The Medical Social Worker must possess current knowledge of the bio psychosocial aspects of illness, disability and death and be able to apply this knowledge in a professional and understanding manner in direct social work services to patients and their families and in collaboration with all health care professional staff. Has a working knowledge of departmental policies, facility policies and procedures, professional standards, and applicable local, state, and federal laws.

  • COLLABORATION: As a member of the Care Center multidisciplinary team, provides input of residents' social and emotional identifiable needs at Care Conferences and weekly rounds. Is able to facilitate ongoing resident council meetings with residents to ensure their rights are understood (implemented). Maintains contact with residents' representatives (guardian, Power of Attorney, family member) to report on changes in health, current goals, discharge planning, clothing needs and room transfers. Fills in as needed for the Medical Social Work Manager at acute discharge planning meetings and swing bed resident care conferences and provides input as to patients' financial concerns, discharge needs and plans.

  • FINANCIAL RESPONSIBILITY: Monitors residents' funds in the resident trust accounts to ensure residents' Medicaid eligibility is maintained where applicable. Completes annual Medicaid Reviews.

  • RESIDENT/PATIENT CARE: Assists potential applicants in qualifying for Care Center Admission. Provides support and information to families at the time of resident's death. Within a strict timeframe completes Social Work MDS sections and quarterly summaries for residents, as well as, a psycho-social assessment and care plan at time of admission and thereafter annually. Upon request, completes a Social Work Consult for acute patient(s).

  • INFORMATION AND PRACTICAL ASSISTANCE: Receives inquiries and referrals for social services. Screens and secures free or discounted travel for clients who qualify and have off-island medical appointments. Composes letters for clients seeking financial assistance for their medical bills. As time allows assists clients in applying for Social Security Disability and Medicaid Insurance.

  • CUSTOMER SERVICE: Embraces the Providence Service Standards in all aspects of position. Presents and conducts self in a professional manner. Provides support to coworkers and customers while demonstrating service excellence by building positive interpersonal relations to ensure productive and positive relationships are maximized. Develops a deep understanding of customer's perspectives and needs and works to meet their expectations. Builds a culture of true caring by behavior and interactions with others.

  • COMMUNICATION/TEAMWORK: Communicates in a compassionate and professional manner. Demonstrates professional interpersonal and verbal skills when communicating with other department and of the care delivery team to provide optimal continuity of care to patients and their families. Is able to communicate effectively with a wide variety of people from differing literacy levels, languages and cultures. Demonstrates commitment to teamwork and interacts harmoniously with others to ensure team success. Supports new orientees in learning department routines.

  • Ensures the Medical Social Work Manager is well informed about social work activities and any problems concerning patient or resident care.

  • CRITICAL THINKING AND ORGANIZATION: Demonstrates effective priority setting and organization skills to promote departmental workflow and cost efficiency. Ensures smooth, efficient services and maximizes comfort and confidentiality for the patient. Anticipates potential obstacles and adjusts work assignments to prevent unnecessary delays. Recognizes errors/problems and takes corrective action when necessary. Utilizes available resources to recognize errors/problems and takes corrective action and assists with problem resolution.

  • QUALITY IMPROVEMENT: Supports the quality control and quality assurance programs by supplying and/or developing indicators to measure, monitor, and report performance to other team members. Seeks input from various customer groups to maintain quality. Reports any performance improvement suggestions to the department leader.

  • PURCHASING: Ensures an effective purchasing process is maintained by requisitioning necessary products and supplies for personnel, determining necessary quality/quantity requirements, selecting suppliers according to current contracting, arranging for deliveries, following up on orders, dispensing of equipment and other items when necessary. Develops and/or maintains department records keeping system, regarding par levels of department supplies. Submit invoices for approval and processing in a timely manner.

  • EDUCATION/PROFESSIONAL DEVELOPMENT: Assist with responsibility for obtaining information and expanding job knowledge and competency that reflects current practice. Role models personal development and education. Evaluates own practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations.

  • Assists in training of non-social work employees in areas of expertise, i.e. elder abuse and neglect, end of life decisions, and identifying spiritual needs.

  • PERSONNEL: Demonstrates knowledge and support of quality improvement goals. Participates in department, facility and organizational projects or committees as assigned.

  • Implement care/services that recognize age/diversity specific needs/issues of customers served.

  • Completes Competency Plan for assigned job and department.

Qualifications:

Required qualifications this position include:

Education:

  • Bachelor's Degree in Social Work.

License/Certification:

  • Current Social Worker license with the State of Alaska required.

  • If employee has worked under the Social Worker title for Providence since 2001, licensure is not required.

  • Must maintain current licensure for appropriate level of education.

Experience:

  • Must have experience with patient and family assessment with underserved populations to include the chronically ill.

Preferred qualifications for this position include:

Education:

  • Master's Degree in Social Work.

License/Certification:

  • Current Social Worker license with the State of Alaska required.

  • If employee has worked under the Social Worker title for Providence since 2001, licensure is not required.

  • Must maintain current licensure for appropriate level of education.

Experience:

  • 2 years Social Worker experience in a hospital setting experience.
