The Providence Alaska Medical Group palliative care department, serving patients in both the inpatient (Providence Alaska Medical Center) and community setting, is seeking a master’s prepared Social Worker to join our clinical team. Under general supervision the role of Social Worker is to assist in our compassionate person-centered care of patients and families facing serious illness in support of their quality of life, comfort and decision making (goals of care and advance care planning.)

Provides grief counseling, goals of care counseling, and patient/family psychosocial support for both inpatient and outpatient, individual, couples, group and/or family services to improve or maintain the social, emotional, functional, and physical health status of the client. Works in collaboration with client and/or family, physician, other staff and other community services to assess, plan, implement, and evaluate appropriate treatment.

Job responsibilities are as followed below:

Perform psycho-social-spiritual assessments of patient’s facing serious illness, assist in Advance Care Planning and end-of-life decision making.

Perform “Serious Illness Conversations, provide grief and bereavement counseling and aid in connecting patients and families to community resources.

Biopsychosocial assessment: Past/current health conditions, family structure, roles, patterns/styles of communication and decision making, developmental issues, spirituality/faith, cultural values/beliefs, social supports, past, experiences with death, loss, debility.

Serious Illness Conversations: Understanding, goals, hopes, worries/review of goals

Disease specific Advanced Care Planning: Education and completion of POLST and Advanced Directives, EOL planning/education – grief / Bereavement support -risks for complicated grief.

Required qualifications:

  • Master's Degree in Social Work.

  • Alaska Clinical Social Worker License upon hire.

  • 2 years experience in providing psychotherapy or similar experience.
