Providence Health & Services Social Services Clinician (BSW) - PBMG Clinic in Anchorage, Alaska
Providence is calling a Social Services Clinician (BSW) (Providence Behavioral Health Clinic, Day Shift, 1.0 FTE) at Providence Alaska Medical Center in Anchorage, Alaska. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will:
Provide age and culturally appropriate direct service to a diverse socio-economic population with a variety of social service needs.
Utilize problem solving skills, referrals, interventions and information integrated with knowledge of the bio economic psychosocial aspects of illness, disability and death to assist clients and providers with social service concerns.
Provide advocacy as needed to encourage participation from clients in resolving problems.
Assist client in accomplishing tasks necessary to access services such as filling out appropriate forms, obtaining necessary documentation or authorizations.
Contribute to the plan of care as established by the inter-disciplinary team and works to obtain the necessary services and resources related to the patient’s need.
Complete a daily schedule, as necessary, including the documentation of every client encounter according to clinic policy.
Report any unusual incidents to their supervisor and/or clinical provider(s) and report to the state any suspected case of child/elder abuse or neglect as mandated by DHSS.
Required qualifications for this position include:
Bachelor's Degree in Social Work
Eligible for Bachelor's of Social Work (BSW) Alaska State licensure
1 year work or practicum experience in the provision of social services
Basic computer skills and Microsoft Office Word and Outlook
Preferred qualifications for this position include:
Knowledge of the community resources
Knowledge of EPIC or other electronic health record
