Providence is calling a Registered Nurse (RN) Hospice Clinical Resource (Hospice, Day Shift, 1.0 FTE) to Providence Hospice in Anchorage, Alaska. In this position you will:
Using the nursing assessment and process, make an assessment of the patient and family’s needs to develop the nursing care plan. Initiate the admission and plan of care. Confer with physicians and other hospice team members to develop the interdisciplinary Hospice plan of care.
Demonstrate respect, compassion and cultural sensitivity to patients/families, other employees and students.
Advocate for excellence in patient care.
Utilize sound judgment and applies knowledge of process, systems, and skills in the performance of duties.
Demonstrate effective communication and collaboration skills when interacting with patients/families, other health care team members and members of other departments. This communication includes situation, background, assessment, and recommendations (SBAR). Utilizes tools adapted through quality and safety initiatives when communicating patient status.
Teach, supervise, and counsel the patient and caregivers regarding patient care in accordance with the patient and family’s wishes including cultural and religious practices, emotional barriers, desire and motivation to learn, physical and/or cognitive limitations, communication styles, and language barriers.
Maintain patient clinical, progress, and itinerary records to meet JCAHO and Federal regulations.
Participates as preceptor in the orientation of new staff and/or students as assigned by supervisor.
Evaluate and instruct staff and students in the performance of direct patient care and clinical documentation
Actively participate in the development, implementation and evaluation of unit-based orientation and skills competency assessments while following best practices.
Required qualifications for this position include:
Graduate of an accredited/approved school of Nursing
Current Alaska RN license required
Alaska State Drivers license required and proof of automobile insurance according to AK State Vehicle laws.
Basic Life Support (BLS) certification required from American Heart Association (AHA) upon hire
2 years clinical experience
Preferred qualifications for this position include:
1 year experience in a Medicare certified hospice or community health nursing
