Providence RN Case Manager - Family Medicine Center in Anchorage, Alaska
Providence is calling a Registered Nurse (RN) Case Manager (Family Medicine Center, Day Shift, 1.0 FTE) to Providence Physician Service Organization in Anchorage, Alaska. In this position you will:
Coordinates transitions of care to ensure appropriate follow up.
Utilizes multidisciplinary care team model to manage a core panel of patients. Collaborates with other teams to share and establish best practices. Manages acute and chronic care needs of the patients.
Displays behavior consistent with professional nursing standards and policies.
Provides relevant case management expertise to committees and participates in quality improvement activities.
Assists in implementing systems into the practice for increased efficiency.
Participates in department and team meetings and committees as assigned.
Performs other duties as assigned by the Medical Director and/or Clinic Manager.
Completes Competency Plan for assigned job and department.
Required qualifications for this position include:
Graduation from an accredited school of nursing OR Bachelors Degree in Nursing
2 years professional nursing experience
Licensed as a Registered Nurse in the State of Alaska
Preferred qualifications for this position include:
Prior clinic or outpatient experience
