Providence Physical Therapist - Home Health in Anchorage, Alaska
Providence is calling a Physical Therapist (Day Shift, 1.0 FTE, 40 hours/week) to Providence Home Health in Anchorage, Alaska. In this position you will:
Assess, plan, implement, and evaluate physical therapy under the prescription of a physician licensed in the State of Alaska.
Communicate with team members and outside agencies to identify patient needs, changes, overall condition of patient, and determine future levels of care.
Contact vendors for durable medical equipment needs.
Assist in coordinating discharge plan.
Document evaluation, plan, care delivered and outcomes according to hospital and department standards.
Provide patient, family and community education.
Implement care/services that recognize age/diversity specific need/issues of customers served.
Demonstrated safe use and knowledge of equipment in home department.
Complete initial and annual Competency Plan for assigned job and department.
Required qualification for this position include:
Bachelor's Degree OR Master's Degree OR Ph.D.
Current license to practice Physical Therapy in the State of Alaska.
Basic Life Support required upon hire.
Preferred qualification for this position include:
Credentials in sub-specialty
Experience in sub-specialty
