Medication Reconciliation Technician - Pharmacy 0.2 FTE, Variable at Providence Alaska Medical Center

The medication reconciliation technician is responsible for compiling an accurate prior to admission medication history under the direction of a pharmacist. Information will be obtained by interviewing the patient, family, or caregiver, calling pharmacies, and using other available resources. The information obtained will be entered into the electronic medical record (EMR) and it will be used by physicians and other health care providers to determine medication orders and treatment plans during and post discharge from the hospital.

Qualifications:

Required qualifications:

  • Licensed as a Pharmacy Technician by the State of Alaska

  • Two (2) years experience in a pharmacy department

Preferred qualification:

  • Some college
