Providence Health & Services Medication Reconciliation Technician (part time) in Anchorage, Alaska
Providence is calling a Medication Reconciliation Technician (Pharmacy,Variable Shift, 0.2 FTE) to Providence Alaska Medical Center in Anchorage, Alaska. In this position, you will:
Understand the medication reconciliation process in the EMR.
Collect, clarify, create, confirm, and document accurate information regarding patients’ home medication.
Work with pharmacists to coordinate daily work, identify issues needing a pharmacist’s attention, drug and disease state information, and help troubleshooting.
Establish effective working relationships with healthcare staff and providers.
Participate in quality improvement and medication safety projects pertaining to admission medication histories or medication reconciliation.
Facilitate medication charge reconciliation under the direction of the Pharmacy Compliance Officer.
Responsible for running daily reports of un-reconciled dispenses from pharmacy as well as automated dispensing cabinets.
Access patient profiles within Epic to distinguish whether an un-reconciled dispense was administered. Confirmed by looking for an associated charge in Epic or review waste reports to determine if the medication in question was wasted by the floor nurse.
Collected information is maintained on spreadsheet and sent to the respective patient care area, Clinical Manager for review and reconciliation.
Responsible for locating trauma sheets in the soft charts for patients that have recently been admitted or seen for a trauma. With the information found on the trauma sheets, the tech is then responsible for locating the associated charges to ensure that the patient was billed correctly for administered medications.
Mentor and train pharmacy staff as needed, regarding obtaining medication histories and documenting in the EMR.
Implement care/services that recognize age/diversity specific needs/issues of customers served.
Complete initial and annual Competency Plan for assigned job and department.
Required qualifications for this position include:
High school diploma or GED
Licensed as a Pharmacy Technician by the State of Alaska
2 years work experience in a pharmacy department
Preferred qualification for this position include:
- Some college
