Providence Health & Services Health Navigator - PMG Senior Care *1.0/Days* in Anchorage, Alaska
Providence is calling a Health Navigator (Pmg Senior Care, Day Shift, 1.0 FTE) to Providence Health & Services in Anchorage, Alaska. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will:
Coordinate care for panel patients before visits, after visits, and between visits and may accompany the primary care provider during the visit, with special focus on patients at higher risk.
Facilitate laboratory testing per standing order routines and scheduling with health providers to assure coordinated continuum of care at PMGA.
Manage referral process from start to finish, including follow up of incomplete referrals. Participates in development of referral tracking workflow.
Coordinate care with outside agencies or case managers as pertinent to patient needs.
Educate patients when appropriate regarding healthy lifestyle and their acute or chronic conditions.
Conduct Risk Assessments for identified populations and discusses strategies to reduce risk with patients based on findings.
Required qualifications for this position include:
- 2 years of care coordination experience
Preferred qualifications for this position include:
Experience in an outpatient setting
