Providence Health & Services Family Care Coordinator - Alaska CARES in Anchorage, Alaska
Providence is calling a Family Care Coordinator (Alaska CARES Clinic, Day Shift, 0.5 FTE) to Providence Alaska Medical Center in Anchorage, Alaska. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
The Family Care Coordinator will be responsible for the coordination of care and mental health referral services of Alaska CARES program, providing assessment and referral for children and families.
In this position you will:
Assist multidisciplinary team to recognize and respond to mental health needs of vulnerable, abused children.
Provide referral and connection to needed services for children and families.
Provide the forensic interview to children and teens when necessary.
Document the history provided by the parents and child in a legal report.
Provide follow up telephone consultation for families and monitor their linkage to services.
Coordinate and provide the agenda for the community multidisciplinary child protection team.
Be responsible for entering into the database, NCAtrak all information, re: children they have worked with.
Required qualifications for this position include:
Bachelor’s degree in social work, psychology, or related health care field
2 years’ experience working with children and families, preferably in child protection system, mental health clinic or agency
Preferred qualifications for this position include:
Experience with OCS, law enforcement, and/or multi-problem families
About the Alaska CARES Clinic:
Alaska CARES is an outpatient clinic located near Providence Alaska Medical Center in Anchorage. The clinic provides sexual and physical abuse evaluations for children, newborn to age 18 years, and 24-hour on-call services for cases that are considered emergent. These cases usually come to the attention of law enforcement or the emergency departments of local hospitals.
